SHN is hiring an Indigenous Patient Navigator (IPN) who will provide culturally relevant service navigation, counselling, support and advocacy for First Nation, Inuit and Métis and Urban Indigenous (FNIMul) patients and families. A key feature of this role is providing culturally relevant counselling, support, and advocacy for FNIMuI patients and families by facilitating and coordinating access to services, addressing cultural/spiritual needs, and networking with FNIMuI and non-FNIMuI partners

Reporting to the Director of Chronic Disease Management, the IPN supports integration with existing navigational and advocacy support currently provided by local community organizations and regional partners. The IPN will also have a broader scope in terms of community engagement, supporting an Indigenous Advisory Committee, policy development, health literacy, and other building blocks to develop an Indigenous Health Program at SHN.
